Informing Patients of their Named & Accountable GP

It is a contractual requirement that GP practices inform patients of their Named & Accountable GP.

Your named and accountable GP will be responsible for your overall healthcare.

If you have any preference to which named GP you prefer, please you can contact us and let us know. We will do our utmost to accommodate your request.

Please note that this does not restrict you from seeing any other clinician at the practice.

Data Extractions & Data Sharing

NHS Digital is working with the Department for Work & Pensions (DWP) to perform a weekly anonymous extract of fit note usage data for collection, storage, transmission and publication by NHS Digital.

Data includes no of fit notes issued, no of patients with a record as unfit/maybe fit to work, duration, gender, health condition, location, and whether adaptions were recommended.

Do I Have A Choice?

Yes! You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances.

If you do not want information to be shared outside this practice, simply inform our reception team.
